The Consulate General of the Republic of Poland in Los Angeles will visit Seattle in November. On Thursday, Nov. 19, and Friday, Nov. 20, applicants will be able to submit Polish passport applications at the Polish Home (Dom Polski). Registration opened on Oct. 19.

The consular visit will run both days from 9 a.m. to 5 p.m. at the Polish Home, 1714 18th Ave. in Seattle. Consuls will accept passport applications only.

Appointments must be booked through the e-Konsulat portal by selecting the Seattle consular visit as the type of service. People applying for their first passport who do not yet have a PESEL number (Poland’s national identification number) should choose the separate option for a first passport without an assigned PESEL. The booking confirmation must be sent to la.dyzury@msz.gov.pl.

For a child’s first passport, a scan of the Polish birth certificate must be emailed. An adult must attach the voivode’s decision confirming Polish citizenship, a birth certificate and, if the marriage took place abroad, a Polish marriage certificate. Applicants must bring the originals with them. Parents applying on behalf of a child must bring valid Polish documents, and at least one parent must have a valid Polish passport or ID card.

Fees are accepted only as a money order or cashier’s check made out to the Consulate General of Poland in Los Angeles. The consulate charges an additional $15 to mail the finished passport back to the applicant. The photo submitted with the application cannot be more than six months old.
